We are pleased that you have selected us as your Primary
Care Provider! Our Clinical Staff are looking forward to working with you as a team
to keep you healthy. Your Primary Care Provider will see you
for almost all your health care needs. This includes
preventive health visits, routine check-ups, and follow-up
for chronic illnesses. (Click

Our Clinical Team will see you for immunizations
(shots), pap tests, scheduling your mammograms, as well as
be available to you if you suddenly become ill. We want to
assure you that we can be reached 24 hours a day, seven days
per week through our main phone numbers. Should you have a life threatening
emergency, however, go immediately to the nearest hospital.
If you have a question about whether or not you should go to
the hospital, please call us to guide you. If you are unable
to call us before going to the hospital or emergency room,
please call us within 24 hours so we can assist you with
your health care needs.
Our staff works closely with doctors and specialists at area
hospitals and medical practices. If it becomes necessary for you to see a
specialist, our providers will coordinate your visit.
