Medical Director/Physician
Small, federally qualified health
center (currently staffed with one physician, one physician assistant)
seeking part-time (12-16 hours weekly, flexible) board certified
physician to assume duties of Medical Director as well as limited
patient caseload. This new position in a very busy family practice
offers great opportunity for the right person to provide leadership as
we move into an expansion phase. Requires management experience, as well
as commitment to health disparities quality improvement initiative.
Outpatient only; some shared after-hours call expected.

Benefits for Employees of
New Horizons Healthcare Community
Health & Wellness Center
Listed below is a brief description
of each employee benefit for which our employees are eligible. Space
does not permit a complete description of all the conditions, provisions
and exclusions of each plan. The descriptions are intended only as a
brief outline of the coverage and do not represent the actual
eligibility for coverage. Interested individuals should ask for and
review the specific booklet describing each benefit.
Health Insurance Plan
New Horizons Healthcare Community Health & Wellness
Center provides an Employee Health Care Program through Anthem Blue
Cross and Blue Shield for all full-time employees. The plan being
offered is the Key Care 30 plan, with Key Care 20 as an
option provided the employee pays the full difference in cost between
the two premiums. The employee is responsible for 15% of the premium
and the full cost of additional family members. The employee selects
the type of membership at the time of employment and pays his/her
portion of the cost based on the type of membership selected.
The monthly premium amounts will be
payroll deducted in two equal amounts monthly. Coverage becomes
effective on the first of the month following the employee’s date of
enrollment in the plan. Coverage under the plan will end on the last day
of the month in which termination occurs.
Dental Insurance
New Horizons Healthcare provides an Employee Dental
Care Program for all full-time employees through Delta Dental Plan of
Virginia. The company will pay the cost for an employee and the
employee will pay the balance for any dependents.
The monthly premium amounts will be
payroll deducted in two equal amounts monthly. Coverage becomes
effective on the first of the month following the employee’s date of
enrollment in the plan. Coverage under the plan will end on the last day
of the month in which termination occurs.
Life Insurance Plan:
New Horizons Healthcare provides Group Life and
Accidental Death & Dismemberment Insurance for all full-time employees
through the Guardian Life Insurance Company. The amount of the coverage
is equal to one time the employee’s basic annual earnings rounded to the
next higher $1,000. (There are certain benefit reductions beginning at
age 65). Coverage terminates upon retirement. Further information on
the Group Life and Accidental Death & Dismemberment Insurance is
contained in the booklet given to each employee when he/she becomes
eligible. Coverage under the plan will end on the last day of the month
in which termination occurs.
Short - Term Disability
Insurance Plan
New Horizons Healthcare offers short-term disability
insurance coverage for all full-time employees through Shenandoah Life
Insurance. The benefit is 60% of weekly earnings. Benefits begin the 1st
day after an accident and the 8th day after an illness and
continue for a maximum of 180 days.
VALIC
403(b)
New Horizons Healthcare offers participation in a
403(b) annuity investment plan, with an employer contribution of 3% of
salary upon completion of 1000 hours of employment. No employee matching
contribution is required, but the option to contribute is available upon
employment.
