Fees

New Horizons Healthcare is not a free clinic.  Our fees are comparable to other health care providers in our area.  Sliding fee discounts are provided to eligible patients, depending on income level.

New Horizons Healthcare also accepts Medicare, Medicaid, FAMIS, and most private insurance plans.

Also, all eligible Medicare patients who use New Horizons Healthcare for medical treatment are not required to pay the yearly deductible.  This is another advantage of receiving your primary care at New Horizons Healthcare.

Although reduced charges are available, patients are responsible for all fees incurred as a result of services received.  Payment is expected at the time of service unless special payment arrangements have been made.

How to Qualify for Sliding Fee Discounts

If you are interested in becoming a patient of New Horizons Healthcare, you will be offered an appointment with the Eligibility Coordinator, Teresa McCadden, for patient orientation and financial review prior to your first provider appointment.

Teresa will discuss the financial and appointment policies with all new patients in addition to determining eligibility for the sliding fee discount.  Eligibility is based on your income and family size and is good for one year.  Some insured patients, including Medicare, may also qualify for a sliding fee discount.

New patients will be mailed a New Patient Packet to include the following documents and you are advised to bring these completed documents to your first appointment with the Eligibility Coordinator:

• Welcome Letter (Adult or Pediatric as appropriate)
• Patient Registration Form
• Medical History Form
• Sliding Fee Discount Application form
• Sliding Fee Waiver Form
• Consent to Treat Form
• Medical Records Release Form
• Insurance Card for any policies you have.
